Enrollment and Eligibility

Many people are uninsured and are unaware that they are eligible for low- to no-cost health care programs. Our knowledgeable and experienced Enrollment and Eligibility Department staff will help you understand and navigate the enrollment process of many low- to no-cost health care programs including, Family PACT, CMSP. Medi-Cal, Medicare, Healthy Families, and more.

Remember:

• Please be sure to bring your proof of residency, income and photo I.D. to your enrollment and eligibility appointment.
• We offer translation services upon request.
